If you are expecting a newborn, new to our area or transferring your child’s care, Legacy Pediatrics would be happy to provide you a medical home. We accept most major insurance plans, including, Blue Cross, Blue Shield, United Healthcare, Vantage, Aetna and Cigna. We do NOT accept any of the Louisiana Medicaid plans.

• Bring to New Patient/Newborn appointment: Insurance card, new patient registration form, medical history form, medical records if available.
• Release of Information Form to transfer care.
